Forms for the 2025-26 Academic Year

These forms may be requested by our office or could be optionally submitted for a reconsideration of financial aid eligibility. Carefully read the information on the form, provide all requested supplementation documentation (as applicable), and be mindful of the signature certifications. After an initial review of your submission, additional documentation and/or clarification may be requested.

Additional Financial Aid Forms
  • Appeal for Special Circumstances Consideration (PDF) - Report any unforeseen changes in family financial situations not addressed by the FAFSA Filing. 
  • Computer Expense Request Form (PDF) - This form will be available for submission 60 days prior to the start of the Fall 2025 semester.
  • Dependency Status Worksheet (PDF) - Only submit this form if requested. This form is used to clarify a student's response to the Student Personal Circumstances Section on FAFSA (Section 5).
  • Identity and Statement of Educational Purpose (PDF) - Only submit this form if we have specifically requested it. This form must be mailed or completed in person; it cannot be accepted by fax or digital upload.

Be sure to include the student's name, W&M ID number and Passphrase (parents and students will have different passphrases) in all correspondence.
