Student Attestations

New Federal regulations (effective July 1, 2024) prohibit William & Mary from enrolling distance education students in a professional licensure preparation program if the program at W&M does not meet the educational requirements for licensure in the student's state.

 Students who intend to seek licensure and employment in a different state may be eligible for enrollment in their chosen program, providing that their chosen program meets the educational requirements for licensure in that different state and provided that they submit a formal Student Attestation.

A student attestation is a formal notification by the student to W&M clarifying that they intend to seek licensure and employment in a state other than their location at initial enrollment.

How to Submit a Student Attestation
  1. Email Assistant Provost Matt Smith to request a Student Attestation Form. Be sure to include your full legal name and the name of the program to which you are applying.
  2. Assistant Provost Smith will send you a DocuSign form to complete. The email will come from DocuSign.
  3. Complete and submit the DocuSign form within 14 calendar days of the date on the email.
  4. The form will then be reviewed by W&M. You will receive a copy once the review is complete and a decision has been made.
