Description: CDC created this helpful interactive dataset that includes information on health risk behaviors, health outcomes, health insurance, demographics, and preventive healthcare use. Data are available for all 50 states and D.C. Users may sort and analyze the data from different views. The “Summary View” allows users to analyze 33 indicators for each state that are divided into four categories: death rates, health burden, risk factors, and preventive services. The “Indicator View” allows users to analyze the 33 indicators for each state by total/rate, race/ethnicity, age, gender and trend (rate), when possible. Users may download the data into a CSV file by clicking on “Export to CSV file” at the bottom of each viewed page. The indicators include the following and show the years available:
Death rates: infant mortality (2000-2014), heart disease (2000-2015), stroke (2000-2015), suicide (2000-2015), homicide (2000-2015), drug poisoning (2007-2015), motor vehicle (2001-2015), and cancer (2000-2015)
Health burden: HIV diagnosis (2008-2014), Hepatitis B and C cases (2005-2015), CLABSI – standardized infection ratio (2009-2014), adult obesity (2001-2016), youth obesity (2001-2015; biannual), medicated hypertension (2001-2013; biannual), teen birth rate (2001-2014), diagnosed diabetes (2000-2014), diagnosed high cholesterol (2001-2013; biannual), and diagnosed hypertension (2001-2014; biannual)

Description: Data include information on births, infant deaths, children, childhood immunizations, adulthood immunizations, deaths, life expectancy, child deaths, teen deaths, cancer, diabetes, heart disease, smoking, asthma, obesity, physical activity, mental health, sexually transmitted diseases, oral health, deaths due to firearms, firearms and children, disability, and influenza and pneumonia. Each of these categories has its own subcategories providing information on each state.
To download multiple years of data for each state, users need to click on the website/data file link below, click on the appropriate indicator, select year under “Timeframe” on the left if available, click on the “Trend Graph” tab if available, and click on “Download Raw Data” tab. In some case, the user will only be able to download one year of data; in other cases, multiple years are available.

Mortality Measures
Data Title: Linked Birth/Infant Death Records

Description: These data provide count and rates of deaths for children under 1 year of age, occurring within the United States. Information from death certificates is linked to corresponding birth certificates. Data are available by county of mother's residence, child's age, underlying cause of death, gender, birth weight, birth plurality, birth order, gestational age at birth, period of prenatal care, maternal race and ethnicity, maternal age, maternal education and marital status. The data are produced by the National Center for Health Statistics.
To access the data, agree to the terms and conditions. Then use the search function to select which variables for which you wish to obtain data. After sending the request, the data should appear in a table, which can then be exported into a text file.

Description: Data provide information on the mortality rates among Medicare enrollees in all states for each year from 1999-2013. Some of the variables included are state, number of Medicare enrollees, total mortality (ASR-adjusted % of deaths among Medicare enrollees), and non-HMO mortality (ASR-adjusted % of deaths among Medicare enrollees without HMO coverage).

Data Title: Multiple Cause of Death

Description: In discussing mortality data from the National Vital Statistics System, the CDC notes: “Cause-of-death data are traditionally presented in terms of one underlying cause for each death. However, underlying-cause data can be augmented with additional information on the other conditions that the medical certifier reported as contributing to death…Because several chronic conditions are often reported, multiple-cause data may be important in chronic disease surveillance.”
The Multiple Cause of Death dataset contains information on both the underlying cause of death as well as other contributing causes of death for U.S. counties. Data are based on death certificates for U.S. residents. Each death certificate contains a single underlying cause of death, up to 20 additional multiple causes, and demographic data. The number of deaths, crude death rates, age-adjusted death rates and 95% confidence intervals for death rates can be obtained by cause of death (4 digit ICD-10 codes, 113 selected causes of death, 130 selected causes of infant death, drug and alcohol related causes of death, injury intent and injury mechanism categories), place of residence (national, region, division, state, and county), age (single-year-of age, 5-year age groups, 10-year age groups and infant age groups), race (American Indian or Alaskan Native, Asian/Pacific Islander, Black or African American, White), Hispanic ethnicity, gender and year. Data are also available by place of death, month and week day of death, and whether an autopsy was performed.

Data Title: Underlying Cause of Death (Detailed Mortality)

Description: The National Vital Statistics Reports notes: “Cause-of-death statistics are based on the underlying cause of death.” Unlike the dataset immediately preceding this one, this database provides detailed information only on the Underlying Cause of Death (and does not include multiple causes of death) for residents in U.S. counties. Data are based on death certificates. Each death certificate identifies a single underlying cause of death and demographic data. The number of deaths, crude death rates or age-adjusted death rates, and 95% confidence intervals and standard errors for death rates can be obtained by place of residence (total U.S., region, state and county), age group (single-year-of age, 5-year age groups, 10-year age groups and infant age groups), race, Hispanic ethnicity, gender, year, cause-of-death (4-digit ICD-10 code or group of codes), injury intent and injury mechanism, drug/alcohol induced causes and urbanization categories. Data are also available for place of death, month and week day of death, and whether an autopsy was performed.

Description: Data include information on health coverage status by age, gender, and various income levels. The data are broken into many categories, which include: health insurance status (2013-2016), health insurance status by FPL (2016), health insurance status by gender (2016), nonelderly uninsured (2013 or 2014), nonelderly with employer coverage (2015 or 2016), nonelderly with Medicaid (2015 or 2016), Medicare coverage (2016), and private sector coverage (2016). Each of these categories has its own subcategories providing information on each state.

Description: Health Expenditures by State of Provider present estimates of health care spending by type of establishment delivering care (hospitals, physicians and clinics, nursing homes, etc.) and for medical products (prescription drugs, over-the-counter medicines, and sundries and for durable medical products such as eyeglasses and hearing aids) purchased in retail outlets. Source of funding estimates by state are also provided for Medicare and Medicaid. These estimates are useful in measuring health spending's role in a state's economy. These estimates should not be used to calculate estimates of spending per person in a state (see state of residence estimates).

Data Title: Care of Chronically Ill Patients During the Last Two Years of Life

The data are separated into all chronically ill patients and those with cancer. A few of the variables included in the data are: number of deaths among chronically ill patients living in the state, hospital care intensity index (standardized ratio of inpatient days and inpatient visits), total Medicare spending, total Part B spending, and total standardized FTE physician labor.

Data Title: Hospital Discharges and Post-acute Care

Description: The discharge data include information on selected surgical discharge rates and selected medical discharge rates. For the dataset on selected surgical discharge rates, a few of the operations included are: abdominal aortic aneurysm, back surgery, coronary angiography, coronary artery bypass grafting, hip replacement, knee replacement. For the dataset on selected medical discharges, a few of the medical conditions included are: convulsions, asthma, congestive heart failure, kidney/urinary infection, and diabetes.
Data on post-acute care provide information on individuals upon being discharged from hospitals. The data include 30-day readmission rates, emergency room visits within 30 days of discharge, and follow-up visits within 14 days of discharge.
