Completing the DS-7002

The DS-7002 is a Training/Internship Plan that is a vital part of the Student Intern Program. The parameters of this program are set the by State Department, and it is important that this form is completed fully and accurately.

Form DS-7002 -- check that the form is valid by looking at the "Expiration Date" at the top of the form.
You will submit this in the Student Intern Request in iStart.

- For paid interns
  - Does your Worker's Compensation Policy cover exchange visitors? Select "Yes"
- For unpaid interns
  - Does your Worker's Compensation Policy cover exchange visitors? Select "No, but equivalent coverage"
- Number of FT employees onsite at location: The information is available in the Faculty/Staff Fact Book.
- Annual Revenue: Select “25 million or more,” as confirmed by this report.
