Student Requirements
All full-time students must fulfill both of the following requirements.
1. Health Requirement
The health requirement provides the Student Health Center with a medical history and official immunization record for each student. It also serves to alert the Health Center to the unique medical needs of each particular student. Non-urgent services will not be rendered until all health requirements have been met.
Necessary Information
A physician appointment/physical is NOT necessary. An official copy of your Immunization Record IS required to complete the health requirement. This record is required to be in English and must contain the following immunizations:
- Two M.M.R. Vaccines (after your first birthday).
- Three Hepatitis B Vaccines (or signed waiver).
- Tetanus Vaccine (within the past 10 years).
- Meningitis Vaccine (after your sixteenth birthday/or signed waiver).
- Poliomyelitis Vaccine (3 dose childhood series or 1 dose of IPV after eighteenth birthday).
Deadlines
- Entering fall semester: submit by June 1
- Entering spring semester: submit by January 10
How to Submit
The health requirement is to be completed via your Online Health Portal. Omission or misrepresentation of pertinent medical information is a violation of the honor system.
Fees and holds for late submissions
Failure to complete this requirement by September 15 for fall admission or February 28 for spring admission will result in a late fee of $100. Additionally, a late fee of $150 will be charged should this necessary document remain incomplete.
During this process, a hold will also be placed on your Banner account for noncompliance. This will prohibit you from registering for classes. The information required from you is based on Virginia State Law, William & Mary requirements, and information needed to assist in caring for you.
2. Insurance Requirement
All incoming and continuing, full-time students are required to have health insurance throughout the school year as a condition of enrollment. Every academic year, these students must either elect to enroll in or waive out of the W&M-sponsored Student Insurance Plan to comply with this requirement. Falsifying or misrepresenting insurance information is a violation of the Honor Code and will be reported.
Waiving the Student Insurance Plan
Students who elect to Waive the Student Insurance Plan because you already have health insurance for the entire academic year must submit an approved Waiver Request by July 31 (or December 31 for the spring semester).
New Students: please note that including a copy of your insurance card with your Health Evaluation Form is NOT sufficient proof of insurance; you must submit an online Waiver Request as your proof of adequate health insurance.
Enrolling in the Student Insurance Plan
Students who elect to enroll in the Student Insurance Plan because you are uninsured, underinsured or do not have access to coverage locally must submit an Enrollment Request by July 31 (December 31 for the spring semester).
Automatic charges if you do not waive or enroll in coverage
Students who neither elect nor waive coverage by July 31 (December 31 for the spring semester) will be enrolled in and billed for the Student Insurance Plan and will be financially responsible for the Health Insurance Policy charges posted to your Student Account. Please note it is your responsibility to verify that your Student Account is appropriately billed for the Student Insurance Plan and notify us immediately if there is an insurance billing error.
