Life changes: we're here to help you navigate it.
Many life events qualify for mid-year coverage. Below are some common life events and how to navigate them with our office. A full listing of qualifying mid-year events (QME's) that affect health coverage can be found on DHRM's QME website.
Within 60 days of the event, employees may make changes to health coverage through the state's Cardinal health benefits system. Employees must also submit required documentation of the event.
Address Change
- Log into Banner Self-Service. Click on Personal Information and then Personal Information View/Update.
- Updates through Banner will interface with other systems, including VRS, health insurance vendors, and retirement providers (TIAA-CREF and Fidelity).
- Please note that health vendors take a few weeks to fully update their systems with address changes.
Legal Name Change
- Complete and submit the Name Change Form (pdf).
- Submit required documentation to support name change.
Retirement
- Complete a new benefits enrollment form through Cardinal.

** Inform our Benefit Specialists of the SSN as soon as the child receives the number. Failure to update this information may result in future denial of claims by the carrier.
