Forms

The OGS uses DocuSign for the signing and processing of nearly all its forms. For assistance with DocuSign, please visit W&M's IT page on DocuSign.

Please note that any forms submitted in DocuSign that have been inactive for at least 30 days will be deleted and the form will need to be resubmitted.

Any remaining PDF forms will continue to be submitted via a W&M email address to dean-gsr@wm.edu

!!NEW!! You can now track any DocuSign form you initiate by following the instructions below:

Before initiating the DocuSign form, please create your W&M DocuSign account.  To do this, login to https://docusign.wm.edu to using your W&M email address and password.  You only need to create your account once, after which, you will be able to track envelopes for any OGS form you initiate.  

After initiating the envelope, filling out the appropriate sections, and signing you will click "Finish."  Then you will receive an email instructing you to sign the form a second time.  After you initial and click "Finish" a second time, you will then be able to track the form and see where it is in the routing process.

After submitting the envelope, and signing twice you can login to https://docusign.wm.edu to track the status of your envelope.  At the top click "Agreements."  You should then see any envelopes in the list that are waiting for other signatures. 

Please refer to the instructions found within each form to help you choose the appropriate form for your circumstance. 

The forms below that are provided as "fillable" PDFs should be downloaded before entering text in the fillable fields; be sure to save the form after entering information in the fillable fields.
